After climbing the rusty, out-of-place staircase leading to the rooftop of the Otherworld's Lakeview Hotel, James Sunderland will encounter either a manifested-by-memory Mary Shepherd-Sunderland, or Maria, who will be dressed head to toe in Mary's clothing - This is determined by the choices made/actions taken by the player, prior to this end-battle

Mary mainly attacks with the use of an elongated, tentacle or whip-like appendage, that seems to retract to an area near the back of her head, possibly inside of her triangular-shaped hood. Provided that James is within reasonable range, #MegaMonsterModeMary can knock him backwards and onto the ground. Just as well, the creature can altogether remove James' feet from the grated flooring below him, by wrapping the tentacle around his throat and lifting him into the air. This second tentacle-based attack deprives James of oxygen, reflecting his asphyxiating his wife.

Mary can also orally spew swarming parties of large moths in James' direction; This attack is more or less just agitating, as it's used when a greater distance is placed in between the player and Mary, and the attack itself slowly drains health while slowing James down.

The moths that surround the cage of, and flow from within Mary's monstrous reincarnated form are symbolic to death, rot and decay. Moths are known flesh-eaters, with the presence of in this situation being symbolic to Mary's illness having basically "eaten away" at several layers of skin on her face and body.

The once-caged moths found inside Woodside Apartment's Room 202 are extensively attracted to James' flashlight, and persistently flutter around him whilst in the apartment, given that his flashlight is on.

Initially, the flashlight is also obtained from Woodside Apartments, inside of Room 205, where it was peculiarly left switched 'on', and placed on the front of a limbless mannequin that has been dressed in Mary's clothing.
Also, within the Toluca Lake Observation Deck men's restroom, a quote from Job 13:28 can be found scrawled across the wall: "While I am decaying like a rotten thing, Like a garment that is moth-eaten."
