First things first, stop wasting your time on people who don't care about you, people who aren't good for you, people that will hurt you in the long run, etc. You need to realize that your happiness and well-being is IMPORTANT and you shouldn't suffer under the hands of someone you know currently or knew.
If you don't get out of the toxic friendship/relationship right now, you will eventually keep suffering until you cannot take it anymore. You need to have people around you that encourage you to do good things and won't talk about you behind your back in a negative manner.

You deserve to be around people who only bring you up, not bring you down
Obviously I know there are many possibilities regarding why you cannot get out of a relationship/friendship but you need to try and have high hopes. If you cannot believe in yourself, tell yourself that you're strong and a badass human being. Toxic people can manipulate you and damage your mental health. If you have a gut feeling that the person you're talking to is toxic and acting very off ( like if they used to be nice but now they're acting like an asshole ) THEN LEAVE.
