Not sure some people suggest its his magic eye covered in blood
Fanon has multiple variations but this one had the biggest impact, which caused people not actually reading the actual comic thinking this is a quick summery of the story, people made multiple animations according this story and even thinking the story IS the canon one which gets easily debunked since the fact that this was not made by The creator, "sour apple studios" is mainly why this story is not true but fanmade many more variations would come to light with this narrative, usually covered in blood, holding an axe or anything of that sort while constantly eating humans which again isnt true sans in horrortale hates eating humans and only hunts them for his brother and the others not probably cus its disgusting or anything but probably because he dosnt want the food to be wasted on him

Early concept was soon to be deleted by sour apple studios and then reed by fans which is the most famous version that includes his weapon we never see ever again after this concept which is a Giant thigh bone sadly this weapon never comes into the shade in the early versions of the character and theres no guarantee that we ever will see it again since there could be a reason why sour apple deleted it
